HEALTHSAFETYANDENVIRONMENTCOMMITTEESECONDQUARTERGENERALMEETINGLocation:LoneStarCollegeDate:22April2010AgendaItem:Opening:JohnKarish,EnscoInternationalHSEManagerandCommitteeChairmanopenedthemeetingwelcomingeveryonetothecommitteemeeting.JohnthenthankedLoneStarCollegeforhostingthemeetingandintroducedMeganCostanza,InterimDirector,LoneStarCollegeSystemwhoprovidedsafetybriefingandgeneralbuildinginformation.JoeHurtreviewedtheIADCAntiTrustStatement.Johnthenaskedeveryonetointroducethemselves,seetheattendancesheet.JohnKarishledthegroupinamomentofsilenceforthoseinvolvedintherecentseriousincidentsthathaveoccurredintheindustry.AgendaItem:LoneStarCollegeWorkforceDevelopmentBonnieDean,Director,BusinessDevelopmentandSalesandMeganCostanza,LoneStarCollegemadeapresentationonthegoalsoftheschoolandopportunitiesforworkingwithindustry.AgendaItem:GeneralDiscussion:Theattendeesdiscussedanumberofissues:1.Drugtestingingeneral,butmostlydrugtestingissuesoftestingthirdcountrynationalsinforeigncountries.2.IncidentwithaSRLwhenamanhadaslowspeedfallagainstthesideofamudtankthanslidtothefloorofthemudpit.DuetotheslowspeedofhisfalltheSRLdidnotengageandthemanwasinjured.3.Therewasgeneraldiscussiononincidentsandissuesarounddrillcuttingswastehandlingequipment,i.e.augers,conveyersandotherequipment.Oneoftheattendeesdiscussedinstructionsandotherinformationthatisavailablefortrainingrigpersonnelwhohavetoworkarounddrillcuttingswasteequipment.4.Therewasdiscussionregardingincidentswhenpeopleweretextingwhiledriving.Onecompanyhasputcontrolsontheircompanycellphonessothattheycannotreceiveorsendsignalswhilethevehicleismoving.Anothercompanyfoundthatmosttextingbeingdoneoncompanycellphoneswereforpersonalreasonssotheyhaveremovedtextingcapabilityfromallcompanycellphones.Oneattendeereportedthatayoungladywhowastextingwhilewalkingwasstruckandkilledwhenshesteppedinfrontofavehicle.5.Anotherattendeesharedapersonalincidentthatoccurredtoamemberofhisfamilywhoisasupervisorwhowasattemptingtoassistsubordinatesincatchingupwithproductionoutput.Allthehandprotectionwasbeingutilizedbytheworkerssothesupervisorchosetoworkwithouthandprotectionandinasecondcutthetipoftheirfingeroff.Themessageofthisincidentisitonlytakesasecond,evenwhenwearesupervisorsorworkers,whenwetakeariskandfailtofollowcompanyrecommendedprocedurestobecomeinjured.6.Discussionwasheldondrivingwhilefatigued.Afterreviewofincidentsandbiorhythms,onecompanyhasbanneddrivingbetween11:00pmand5:00amtoavoidfatigueddrivers.Theyalsofoundthatbetween2:00pmand5:00pmisanotherhighincidenttimeofday.Onecompanyhascontracted“peoplehaulers”totransportworkerstoremotesites,buthasconcernonwhoismonitoringthefitfordutyofthepeoplehaulerdrivers.AnothercompanyhasbannedtheuseofGPSdeviceswhiledrivingcompanyvehicles.7.Discussionwasheldonthetimeofdaythatincidentsoccur.Onecompanyreferencedbiorhythmsthatindicatethatmostincidentsoccurduringlateeveningtoearlymorninghours.Anothercompanyrepresentativecommentedthattheyfoundthatmostoftheirinjuriesoccurduringdaylighthours.AgendaItem:RegulatoryIssues:AlanSpackmanreportedontherecentissuanceofthefollowingreportsandguidelinesbytheInternationalAssociationofOilandGasProducers(OGP):OGPReport435,AguidetoselectingappropriatetoolstoimproveHSEculture.(http://www.ogp.org.uk/pubs/435.pdf)ThisdocumentprovidesinformationabouttoolswhichcanbeusedtoimproveHSEperformance.Itidentifiescircumsta...
