AppendixR–StandardOperatingProcedureforHotWorksUniversityofMinnesota,FacilitiesManagementNovember2002(Revised:December2006)Page1of15STANDARDOPERATINGPROCEDUREFORHOTWORKS(IncludinginformationabouttheFireandLifeSafetySystemImpairmentProcedure)Department:FacilitiesManagementDate:Dec1,2006Revised:12/2006PURPOSEAHotWorksPermitisrequiredforanytemporaryoperationthatinvolvesopenflamesorproducesheatand/orsparks.Suchoperationsinclude,butarenotlimitedto,brazing,cutting,grinding,soldering,thawingpipe,torch-appliedroofing,andwelding.Thepermitisintendedto1).Preventthebuildingfiredetectionsystemfromaccidentallyactivating2).ControllossesfromaccidentallyignitingUniversityofMinnesotapropertyand3).Ensurethesafetyofallbuildingoccupantswhilehotworksproceduresarebeingperformed.Theseprecautionsareinaccordancewith29CFR1910SubpartQ(OSHA),theMinnesotaFireCode,andNFPASIB.PRIMARYSTEPS1).ObtainaHotWorksPermit2).GainAuthorization3).FillouttheHotWorksPermit4).FollowRequiredPrecautions5).CanceltheHotWorksPermit6).OtherPointsPROCEDURESStep1:ObtainaHotWorksPermit1.1.Beforedoinganytypeofopenflameorhotworksoperation,obtainaHotWorksPermitfromthecustomerservicerepresentativeintheparticularzonewheretheworkwillbeperformed.1.2.Allunitsmustfilloutapermit,includingbutnotlimitedto,ZoneMaintenance,FMConstructionandoutsidecontractors.OutsidecontractorscanhireUniversitySecurityMonitorsorprovidetheirownstaffforfirewatches(whenrequired).1.3Nopermitswillbeauthorizedwhenafiresuppressionsystemisimpaired,andpermitswillbecancelledifanemergencyimpairmentoccurs.AppendixR–StandardOperatingProcedureforHotWorksUniversityofMinnesota,FacilitiesManagementNovember2002(Revised:December2006)Page2of15AppendixKK,Section9oftheStandardsandProceduresforConstructioncontainsthe"RequestforInterruptionofBuildingServices,HotWorksPermit,orFireandLifeSafetySystemImpairment"form.https://cpm.umn.edu/sites/cpm.umn.edu/files/appendixkk_part9_0.pdfStep2:GainAuthorization2.1.Theshift/operationssupervisorshallreviewtheHotWorksPermitanddetermineifandhowtheoperationwillaffectthefiredetectionsysteminthatarea.Ifitdoes,BSACmustbenotifiedforproperinstructions,andthezoneshalldeactivatethefiredetectionsystems.2.2.Theshift/operationssupervisorshallrejectorgrantapproval,anddesignatewhowillbethefiresafetysupervisorforZoneMaintenance,FMConstructionandoutsidecontractors.2.3.Onceauthorizationisgranted,thedesigneeassumestheroleofthefiresafetysupervisor.2.4.ThefiresafetysupervisormustensurethattheprecautionsontheHotWorksPermitarefollowed.Authorizationcannotbegivenunlessallprecautionsaretaken.(Refertothepermitforprecautions.)2.5.Thefiresafetysupervisorshallsignoffonthefinalcheck.(Note:Iftheworklapsesintothenextshift,thefirewatchdesigneeandthefiresafetysupervisormustdelegatetheirresponsibilitiestoasupervisoronthenextshift.)Step3:FillOuttheHotWorksPermit3.1.Fillouttheentirefrontpage(Part1)oftheHotWorksPermit,exceptthefinalcheckitem.3.2.Thefiresafetysupervisorandtheshift/operationssupervisorshallreviewtheHotWorksPermitforthebuilding.3.3.Thefiresafetysupervisormustpostthetopcopy(Part1)inthezoneofficefordocumentation(insurancepurposes).3.4.Thetradespersonperformingthehotworksoperationshallpostthebottomcopy(Part2)ofthepermiton-site.Step4:FollowRequiredPrecautions:AllprecautionsthatarecheckedonthepermitinthespaceprovidedmustbefollowedasperNFPA51B.Step5:CanceltheHotWorksPerm...
