Appendix3TheHumanFactorsRoadmap12COMAHCompetentAuthorityInspectingHumanFactorsatCOMAHEstablishments(OperationalDeliveryGuide)HumanFactorsDeliveryGuide–Version1.0April2016Page2of21Introduction1.ThisDeliveryGuide(DG)supportstheCompetentAuthority’s(CA)programmeofregulatingmajorhazards,byestablishingaclearframeworktoinspecthumanfactors(HF)atCOMAHestablishments.ItbuildsontheHIDRegulatoryModel1andtheprinciplesthatdeterminehowHIDdirectsresourcestoactivitiesthatgiverisetothegreatestrisk,oraremanagedleasteffectively.2.TheDGwillassistRegulatoryInspectors*whentheyinspectHFaspectsofkeysafetymanagementandriskcontrolsystems(e.g.procedures;competenceassurance;permit-to-worksystems)andprovidethemwithclearguidelinestohelpdecidewhenspecialistHFsupportmayberequired.Inaddition,itaimstotargetHFspecialistresourcewhereitisneededmost,byidentifyingprioritytopicsforinspectionbyHFSpecialistInspectors.3.TheDGwillalsoenableRegulatoryandHFSpecialistInspectorstoratetheperformanceofCOMAHoperatorsagainstsuccesscriteriaforanumberofkeyHFinspectiontopics.4.AlthoughtheDGisaimedprimarilyatRegulatoryandHFSpecialistInspectors,itwillhelpCOMAHoperatorsprepareforHFinspectionsandunderstandtherationalebehindthoseinterventions.ThekeytopicsandunderlyingHFprincipleswillalsoberelevantduringHFinterventionsatotherhigh-hazardsitesthatarenotcoveredbyCOMAH.Justification5.TheCOMAHCompetentAuthorityIntelligenceReviewGroup(CAIRG)analysedevidencefromlossofcontainmentsinthechemicalsectoracrossseveralyearsupto2010.Asaresultofthosefindings,the2011HSEChemicalsSectorStrategyidentified‘maintainingcompetence’asakeypriority.This,inturn,ledtothelaunchoftheCA’sDeliveryGuideforinspectingcompetencemanagementsystems2andaprogrammeofinspectionatCOMAHsitesrunningfrom2012to2015.Thatprogrammeisnowdrawingtoaclose.6.Inafollow-upanalysisofdangerousoccurrencesandlossofcontainmentincidentsinitiatedbyhumanandsafetymanagementsystemfailures(upto2014),CAIRGidentifiedawiderangeofHF-relatedunderlyingcauses.Manyofthosecausesoccurredmorefrequentlythanfailingswithcompetencemanagementsystems(e.g.pooroperationalandmaintenanceprocedures;inadequateriskassessment;poorplantandprocessdesign;failingsinsafety-criticalcommunication,includingshifthandoverandpermit-to-work).7.Consequently,whilsttheCArecognisesthatcompetenceassuranceremainsanimportantPerformanceInfluencingFactor3(PIF),andacknowledgesthattheoutgoingcompetenceDGcontinuestosecureimprovementstocompetencemanagementsystemsatCOMAHestablishments,thisnewDGaimstoestablish,andresource,aninterventionprogrammethataddressesabroaderrangeofkeyHFtopics.Thosetopicshavebeeninformedbythehigh-priorityHFissuesemergingfromthe2014CAIRGanalysis.*ThroughoutthisDG,theterm‘RegulatoryInspector’referstoeitheranHSERegulatoryInspector,anONRInspector,oranAuthorisedPersonworkingonbehalfoftheappropriateagency(EA,SEPAorNRW),actingtofulfiltheirresponsibilitiesastheCompetentAuthority.HumanFactorsDeliveryGuide–Version1.0April2016Page3of21Background8.Humanfactorshavebeendefinedasthecharacteristicsofthejob,individualandorganisationthatinfluencehumanperformance.FurtherguidanceisavailableincorepublicationssuchasHSG484andonthehumanfactorshomepageoftheHSEwebsite.9.TheGeneralDutyunderCOMAHrequiresthateveryoperatormusttakeallmeasuresnecessarytopreventmajoraccidentsandlimittheirconsequencesforhumanhealthandtheenvironment.Whererelianceisplacedo...
